Hospice team members work closely with a patient’s physician to provide comprehensive, individualized care in the comfort their home or at our inpatient hospice facility located in Durham.

New graduate nurses can apply for the Post-Acute Transitional Care @ Home (Patch) Program supporting new nurses in their first year of practice as they rotate through home health, hospice, home infusion, and care coordination service lines.
