General Description of the Job Class

Performs specialized health information activities necessary to organize, maintain, and use electronic patient health records. Positions at this level have high customer service, strong analytic and problem solving skills, require interpretation and explanation of policy and internal requirements related to health record documentation and completion, Medical Staff Bylaws, Rules and Regulations and organization-wide functions and policies and procedures. Specific job responsibilities are based on the service unit within Health Information Management assigned and include: Performs HIPAA compliant screening of incoming requests for protected health information of our patients, distribution of medical records, deficiency management to assure compliance with federal and state regulatory and accrediting body requirements for health record content and timely completion and document management to include the capture of health records in an electronic format. Excellent computer skills are required to complete basic to intermediate tasks within Maestro Care, Solarity, Hyland OnBase and other related applications. 

Duties and Responsibilities of this Level

Release of Information Functions:

Performs preliminary screening of authorization (pursuant to State and Federal Privacy Laws), enters requests for information into Maestro Care Release of Information Module, and returns to requestor with explanation if invalid.

Manage tasks associated with the HIM ROI Customer Service office to include greeting visitors, providing guidance for requesting medical records, completing the authorization for disclosure, verifying patient/personal representative authority/identify and facilitates same day customer service by coordination with ROI team.

Serves as front line support through receipt of telephone calls from third party requestors and patients asking for the status of medical record requests.  Assists requestors in understanding the requirements for submission of a HIPAA compliant authorization, provides explanation for canceled requests and responds to general inquiries about obtaining medical records, navigation of MyChart and basic Care Everywhere inquiries.  Requires a high level of communication and customer service skills to appropriately assess and respond to customers while de-escalating issues when necessary.

Responsible for verifying accuracy of patient selection on each page/image of records prepared for disclosure and packages requested records for delivery through paper print, CD or electronic submission.

Performs other duties as assigned

** Position requires onsite presence 65% of time. Rotation to be established by Team Lead. **

Required Qualifications at this Level

Education:

High school or GED diploma required


Experience:

2  years of experience in Health Information Management, (Release of Information, Document Management, Patient Identity, Record Integrity), Coding, or Healthcare Related Field required

Bachelor's Degree in Healthcare, Business, IT, or Related Field can substitute for 2 years’ experience Associate’s Degree in HIT preferred

Knowledge, Skills, and Abilities:

Good organizational skills, excellent investigative and analytical skills with a detail orientation, and strong follow-through capabilities Excellent verbal and written communication skills in order to effectively problem solve, develop working relationships and assist system users. Must be able to meet deadlines, work independently, set priorities and maintain confidentiality. Ability to multi-task and work with a high degree of accuracy and recall. Ability to train and interact with physicians. Ability to work calmly and efficiently in high-pressure situations. Intermediate knowledge of human anatomy and physiology and medical terminology highly recommended. Intermediate skills using Windows-based software, familiarity with information technology hardware and software in a local and wide area network environment
